By submitting this repair request form, you acknowledge and agree to the following terms:
1. Device Inspection: You authorize Myrtle Fix to inspect your device. A diagnostic may be performed to determine the issue.
2. Repair Authorization: No repairs will be completed without your prior approval. Once you approve, charges will apply.
3. Liability Limitation: While we take every precaution, Myrtle Fix is not liable for data loss, further damage due to pre-existing conditions, or damage due to unreported issues.
4. Warranty: Repairs may come with a limited warranty (typically 30–90 days) depending on the part/service. This will be specified at the time of repair.
5. Data Responsibility: It is your responsibility to back up your data before bringing in your device. We are not responsible for any lost data.
6. Payment: Payment must be made in full upon completion of the service, unless otherwise agreed in writing.
7. Unclaimed Devices: Devices not picked up within 30 days of completion may be subject to storage fees or disposal.
8. Customer Communication: You may be contacted via phone or email regarding your repair status, estimate approval, or feedback.
